Non-Food Vendors Application

This form is only for vendors not primarily selling food for on-site consumption and who do not have a storefront in the Upper Madison area. If you need a different application form, review our application options here.

Important Policies

We accept ONLY vendors that sell original art works,
handmade crafts or local food stuffs.
Re-sellers of manufactured merchandise will NOT be accepted.

Fee:

  • There is a $50 fee for participation. You will only be asked to pay that fee if your application is approved.
  • The fee is generally non-refundable. However, if we are legally unable to hold the Street Fair as a result of COVID-related or other restrictions, vendor fees will be refunded.
  • If you plan to provide any food for on-site consumption, we must include you on our Temporary Food Service License and you will need to provide your workers compensation and disability forms as part of your registration. If you do not have those forms, please review the requirements established by Albany County here.
  • Event will be cancelled in the event of heavy rain or dangerous weather only.

Space Details:

  • Space is 10’ x 10’, and located on the street (asphalt)
  • The fair provides an empty space only. You must provide everything else you need (tent/pop-up, tables, signs, etc.)
  • Pop-ups are encouraged. Please plan to weight your pop-up. It can be quite windy at the fair and unweighted pop-ups can be dangerous.
  • Generators are generally not allowed.  If you have any questions about generators, please check with us at vendors@uppermadison.org.

Setup:

  • Set-up no earlier than 10 a.m. and be ready by 11:30 a.m.
  • Event runs 12 to 5 p.m.
  • You must be off the street by 5:30 so it can reopen to traffic

Other Policies:

  • Consistent with NYS law regarding "symbols of hate", vendors at the Upper Madison Street Fair will not be permitted to sell or display any item inscribed with a symbol of hate unless the image appears in a book or similar medium that serves an educational or historical purpose. The term "symbols of hate" shall include but not be limited to symbols of white supremacy neo-Nazi ideology or the Battle Flag of the Confederacy.

What will you do at the Fair
What would you do at the fair if approved?
Please provide enough information for us to determine if you're a good fit for the fair. Much of what you share here will be published on our website, so please describe your offerings in a way that will engage and interest attendees!
Upload a photo for display to the public - it could be your logo or an image of your product, whatever will be most appealing to attendees.

Please describe your merchandise for sale as you want it shared with the public. If you sell packaged food include that here. Food for on-site consumption goes in the next box!
On-Site Food
Food for On-Site Consumption

Will you sell or sample food for on-site consumption at the fair? If you will, choose "Yes" and fill out the other fields in this section. If you will not, choose “No” and go directly to the "Power” section below.

All food offered for on-site consumption is subject to NYS Health Department Regulations. All food must be prepared in a facility that has a permit from the Albany County Health Department or another appropriate regulatory agency. Food may not be prepared at home.

By selecting “Yes”, you confirm that you have read and understood these requirements and that you will follow them.

Please describe any food you will sell or sample for on-site consumption as you want it shared with the public. You must include all food for on-site consumption here so that we can include it on the Temporary Food Service License.

If you are offering food for on-site consumption and you are not already permitted by Albany County, we will add you to our temporary food service permit, but you must provide two forms: 1) Certificate of Workers Compensation Insurance (C-105.2, U26.3, SI-12, or GSI-150.2), and 2) NYS Disability and Family Leave (DB-120.1 or DB-155)
If you have your own health department permit, go directly to the "Power” section below.

If you do NOT have your own health department permit from Albany County, we will add you to our Temporary Food Service License at no extra charge. The county requires that you provide either Form CE-200 - Certificate of Attestation of Exemption, OR both a Certificate of Workers Compensation Insurance (C-105.2, U26.3, SI-12, or GSI-150.2), and a Certificate of NYS Disability and Family Leave Insurance. Power
Power
We have a very limited number of spots with power and an even more limited set of spots where generators are allowed. Please consult with us at vendors@uppermadison.org if you need power or a generator before applying.
Raffle
Raffle Item
Much of the funding for the Fair and other Upper Madison Improvement activities comes from a raffle held the day of the Fair. We appreciate donations of items valued at $25 or more to be used in the raffle. We may combine your item with other items to create appealing raffle baskets. If you are willing to donate a raffle item please describe it here.
Please enter the approximate dollar amount of your raffle item as a whole number with no dollar sign or decimal point. For example, if your item is worth fifty dollars enter 50.
